Expression of wish form

You should use this form to decide who should benefit from your membership of the Scheme, through payment of a lump sum, when you die. This is important as it ensures that the Trustee has sufficient information to make a timely decision as to what happens to any lump sum benefits in the event of your death.

You may have sent the FKI Scheme trustee a form whilst you were a member of that scheme. However, following the transfer of your benefits to the Bridon Scheme, it is important that you inform the Bridon Scheme Trustee of your wishes.
